POSITION SUMMARY

The Case Manager is responsible for case management services for those experiencing chronic homelessness, families experiencing homelessness, or those who are at risk of homelessness. The position works with clients at The Commons, a Permanent Supportive Housing (PSH) community for Veterans, families, and individuals. The role works collaboratively with the Program Manager, property management team, and with the VP of Programs (LCSW) to ensure that all requirements and services of a HUD-sponsored organization are fulfilled. This position is responsible for approximately 15 clients/individuals or a head of household with a family.

DUTIES AND RESPONSIBILITIES

Housing
• Receive referrals from the Pikes Peak Continuum of Care Coordinated Entry system and work on intake and application processes.
• Meet with prospective residents to assess and/or determine eligibility; perform intake and screening services, and conduct review/utilize results from VISPDAT (HUD tool) to aid in pre-selection processes.
• Guide applicants in the application process; obtain necessary documentation and referrals for and/or with assistance for the application.
• Coordinate communication regarding housing needs between clients and Property Managers. Participate in regular property and supportive services team meetings.
• Conduct annual housing recertifications when requested by the independence center and attend annual inspections and lease signings with clients.
• Coordinate move-in and move-out of apartments and conduct home inspections.
Case Management
• Engage with clients and establish rapport with monthly case management meetings. Maintain weekly contact via visits at The Commons and phone.
• Develop individualized service plans for clients and work with each client to set goals for healthy individual growth with an emphasis on self-sufficiency and maintaining recovery; monitor progress in attaining goals; monitor adherence to all Homeward Pikes Peak Housing First policies
• Provide transportation for clients to appointments when necessary and/or connect clients to community transportation resources.
• Assess monthly client engagement and wellness and make appropriate services available as needed.
• Coordinate with other community service agencies to access services for clients as needed.
• Maintain accurate records, which include client charts as well as internal and external reports; maintain confidentiality and privacy per standards with records.
• Contribute to monthly outcome reports.
• Enter data into HMIS and other tracking tools and client files and adhere to data quality goal (90%).
• Participate in the following as needed: Coordinated Entry activities; Housing Navigation; RMHS VFAH (working with veterans), etc., and supervision with immediate Supervisor.
• Perform other duties as assigned.

HOMEWARD PIKES PEAK TEAM RESPONSIBILITIES
• Attend and participate in staff meetings and monthly client luncheons.
• Attend and/or participate in or interact with donors, attend community events, or participate in fundraising as requested.
• Integrate with the overall organization and collaborate with the community.
• Produce documentation in a timely manner as requested for audits and audit resolutions.

KNOWLEDGE, SKILLS, ABILITIES – and MINIMUM REQUIREMENTS
• Associate’s degree in a human services field such as Social Work, Sociology, Human Services, Nursing or a related field.
• Three (3) years of experience in a human services field providing direct client supportive services.
• Education and relevant experience may be interchangeable on a year-for-year basis.
• Lived experience is preferred, with an understanding of challenges related to homelessness, mental illness, and substance abuse.
• Experience working directly with people from diverse socioeconomic and ethnic backgrounds.
• Ability to communicate effectively, perform crisis intervention, and display harm reduction capabilities and trauma-informed care.
• CAS (Colorado Addiction Counselor) or CAT (Certified Addictions Tech) certification is preferred.
• First Aid / CPR certified, or willing to obtain within 3 months of hire.
• QBHA (Qualified Behavioral Health Administrator) certification is preferred.
• Completion of appropriate continuing education requirements every year to maintain licenses and certifications.
• Self-starter and ability to work independently with minimal supervision.
• Strong computer skills required with ability to work with electronic record-keeping applications, use of Excel spreadsheets, including PowerPoint and Word.
• Understanding of HIPAA regulations and standards regarding privacy and confidentiality in the provision of therapy services and medical billing.
• Valid Colorado driver’s license, clean driving record, and must have personal vehicle insurance.

COMPENSATION AND BENEFITS
The pay range for this position is $22 – $24 per hour, depending on experience. This is a full-time, non-exempt position authorized to work 40 hours per week.
