Housing First Case Manager

SUMMARY
The Housing First Case Manager is responsible for Case Management services for those with a dual diagnosis and experiencing homelessness. The Housing First Case Manager will work with clients in a scattered site setting in Permanent Supportive Housing (PSH) for Veterans, families, and individuals who have experienced chronic homelessness. This role works collaboratively with the Housing First Team and reports to the Housing Director (LCSW) to ensure that all requirements and services of a HUD-sponsored organization are fulfilled. This position is responsible for approximately 15 clients/individuals or a head of household with a family.
DUTIES AND RESPONSIBILITIES
Housing
• Meet with prospective residents to assess and determine eligibility; perform screening and intake and services.
• Work with the Housing team and Street Outreach team members to locate the identified person for housing, work on intake processes.
• Guide applicants in the apartment/housing application process; assist in obtaining necessary documentation and referrals and assist with any other items relating to the application.
• Coordinate communication regarding housing needs between client and Property Managers.
• Aid in procuring furniture, home furnishings, etc. when needed, and work with the team to move household furniture to support clients entering housing or maintaining housing.
• Conduct annual housing recertifications, attend annual inspections and lease signings with clients.
• Assisting with move-outs and cleaning units is occasionally required.
Case Management
• Assess biweekly-monthly client engagement and wellness and make appropriate services available as needed.
• Develop individualized service plans for clients. Work with each client to set goals for healthy individual growth, emphasizing self-sufficiency and maintaining recovery.
• Monitor clients’ progress in attaining goals; monitor adherence to all Homeward Pikes Peak – Housing First policies.
• Conduct home visits with each client at a minimum of once a month.
• Maintain weekly-biweekly contact with clients by phone.
• Provide transportation for clients to appointments when necessary and/or connect clients to community transportation resources.
• Coordinate with other community service agencies to access client services as needed.
• Maintain accurate records, which include client records as well as internal and external reports; maintain confidentiality and privacy per standards with records.
• Contribute to monthly outcome reports.
• Enter data into HMIS and other outcome tracking tools and client files and adhere to the data quality goal (90%).
• Participate in regular supervision with immediate supervisor.
• Participate in the following, as needed: Coordinated Entry activities; CHAP; RMHS CACVH (working with veterans), FSC (working with families).
• Perform other duties as assigned.
HOMEWARD PIKES PEAK TEAM RESPONSIBILITIES
• Attend and participate in staff meetings.
• Attend, participate, and interact with donors or fundraising events as requested.
• Produce documentation timely as requested for audits or other business requirements.
• Maintain work and office environment to professional standards.
KNOWLEDGE, SKILLS, ABILITIES and MINIMUM QUALIFICATIONS
• One (1) year of experience providing direct or indirect client support services in the human services field.
• Ability to maintain professional boundaries with clients.
• Knowledge and training in generalized health care standards, substance abuse recovery process and treatment, mental health care, and trauma-informed care skills.
• Experience working directly with people from diverse socioeconomic and ethnic backgrounds, ability to communicate effectively, perform crisis intervention, and display harm reduction capabilities and trauma-informed care preferred.
• First Aid and CPR certification required, or to be obtained within the first 30 days of employment.
• Mental Health First Aid certification by NAMI is required or to be obtained within the first 30 days of employment.
• Experience in general computer skills and Microsoft Office applications, including Word, Excel, and Outlook.
• Understanding of HIPAA regulations and standards regarding privacy and confidentiality in the provision of therapy services and medical billing.
• Valid Colorado driver’s license, clean driving record, and must have personal vehicle insurance.
PREFERRED QUALIFICATIONS
• Lived experience is appreciated with a deep understanding of challenges related to homelessness, mental illness, and substance abuse.
• Bachelor’s degree or similar education in a Human or Social Services field preferred. Relevant experience may be substituted for the educational requirement on a year-for-year basis.
• Peer Recovery Coach certification preferred.
• Addictions counseling knowledge, experience, or certifications preferred.
PHYSICAL DEMANDS
• While performing job duties, the employee will be regularly required to sit and stand for their shift.
• Must be able to move or lift 30 lbs. regularly and heavier on occasion.

COMPENSATION AND BENEFITS
• The salary for this position is $23.00 – $24.00 per hour. This is a full-time non-exempt position.
