Housing First case Manager

(Part-time 20-25 hours per week; option of being Full-time 40 hours per week)

SUMMARY

The Housing First Case Manager is responsible for Case Management services for those with a dual diagnosis and experiencing homelessness.  The Housing First Case Manager will work with clients in a scattered site setting in Permanent Supportive Housing (PSH) for Veterans, families, and individuals who have experienced chronic homelessness.  This role works collaboratively with the Housing First Team and reports to the Housing Program Manager to ensure that all requirements and services of a HUD-sponsored organization are fulfilled.  This position is responsible for approximately 15 clients/individuals or heads of household with a family.

This is a part-time, non-exempt position authorized to work 20 hours per week.

This position has the option of being a full-time, non-exempt position working 40 hours per week.

DUTIES AND RESPONSIBILITIES

Housing

  • Meet with prospective residents to assess and determine eligibility; perform screening and intake and services.
  • Work with the Housing team and Street Outreach team members to locate the identified person for housing, work on intake processes.
  • Guide applicants in the apartment/housing application process; assist in obtaining necessary documentation and referrals and assist with any other items relating to the application.
  • Coordinate communication regarding housing needs between client and Property Managers.
  • Aid in procuring furniture, home furnishings, etc. when needed, and work with the team to move household furniture to support clients entering housing or maintaining housing.
  • Conduct annual housing recertifications, attend annual inspections and lease signings with clients.
  • Assist with move-outs and cleaning units, as occasionally required.

 

Case Management

  • Assess biweekly and monthly client engagement and wellness, and work to make appropriate services available as needed.
  • Develop individualized service plans for clients. Work with each client to set goals for healthy individual growth with an emphasis on self-sufficiency and maintaining recovery.
  • Monitor clients’ progress in attaining goals; monitor adherence to all Homeward Pikes Peak – Housing First policies.
  • Conduct home visits with each client at a minimum of once a month.
  • Maintain weekly-biweekly contact with clients by phone.
  • Provide transportation for clients to appointments when necessary and/or connect clients to community transportation resources.
  • Coordinate with other community service agencies to access services for clients as needed.
  • Maintain accurate records, which include client records as well as internal and external reports; maintain confidentiality and privacy per standards with records.
  • Contribute to monthly outcome reports.
  • Enter data into HMIS and other outcome tracking tools and client files & adhere to data quality goal (90%).
  • Participate in supervision with immediate Supervisor on a regular basis.
  • Participate in the following, as needed: Coordinated Entry activities; CHAP; RMHS CACVH (working with veterans), FSC (working with families).
  • Perform other duties as assigned.

KNOWLEDGE, SKILLS, ABILITIES and MINIMUM QUALIFICATIONS

  • Associate’s degree in a human services field such as Social Work, Sociology, Human Services, Nursing or a related field.
  • Three (3) years of experience in a human services field providing direct client supportive services.
  • Education and relevant experience may be interchangeable on a year-for-year basis.
  • Lived experience is preferred, with an understanding of challenges related to homelessness, mental illness, and substance abuse.
  • Experience working directly with people from diverse socioeconomic and ethnic backgrounds.
  • Ability to communicate effectively, perform crisis intervention, and display harm reduction capabilities and trauma informed care.
  • CAS (Colorado Addiction Counselor) or CAT (Certified Addictions Tech) certification is preferred.
  • Peer Recovery Coach certification preferred.
  • First Aid / CPR certified, or willing to obtain within 3 months of hire.
  • QMAP (Qualified Medication Administration Personnel) certified is preferred.
  • Completion of appropriate continuing education requirements every year to maintain licenses and certifications.
  • Self-starter and ability to work independently with minimal supervision.
  • Strong computer skills required with ability to work with electronic record keeping applications, use of Excel spreadsheets, including PowerPoint and Word.
  • Understanding of HIPAA regulations and standards regarding privacy and confidentiality in the provision of therapy services and medical billing.
  • Valid Colorado driver’s license, clean driving record, and must have personal vehicle insurance.

 

PHYSICAL DEMANDS:

  • While performing job duties, the employee will be regularly required to sit and stand for the duration of their shift.
  • Must be able to move or lift 30 lbs. on a regular basis, and heavier on occasion.

COMPENSATION AND BENEFITS

The salary for this position is $23.00 – $24.00 per hour. T
